Get Cleared To Play With Confidence

Our specialty-trained providers are dedicated to keeping local student-athletes in the game, which is why we are proud to offer FREE sports physicals and well-child checks to ensure athletes’ health and safety for the coming season. 

Additional Details:

  • Cost: FREE
  • Available to student athletes of all ages (grades 6 - 12 and college)
  • Parent/guardian must be present with the student
  • Bring sports physical forms to the physical
  • Students must wear shorts and a T-shirt
  • If a student wears glasses or contacts, they are required to wear them to the physical
  • Well-child checks will also be available by our Valley Medical Center pediatricians

No appointments, walk-ins only. Physicals will be given on a first-come, first-served basis.
